Kenalog is a corticosteroid injection used for inflammatory conditions, including joint pain, allergic reactions, dermatologic conditions, and keloid scars. The administration depends on the condition being treated.
1. Routes of Administration
A. Intramuscular (IM) Injection
Indications: Allergic reactions, arthritis, asthma, lupus, and inflammatory conditions.
Common Dose: 40 mg to 80 mg IM, depending on the severity.
Injection Site:
• Gluteus (buttocks)
• Deltoid (upper arm) – less preferred due to risk of tissue atrophy.
Technique:
• Use a 21-23 gauge needle, 1-1.5 inches long.
• Inject deep IM into the muscle to avoid subcutaneous atrophy.
• Rotate injection sites if repeated doses are needed.
• Inject superficially into the lesion, avoiding excess volume to prevent skin atrophy.
• Repeat every 4-6 weeks as needed.
D. Epidural (Off-Label)
Indication: Severe spinal inflammation, herniated discs.
Common Dose: 40-80 mg per injection
Technique: Performed by a trained specialist under fluoroscopy guidance to ensure accuracy and avoid complications.
2. Precautions & Contraindications
Do NOT use in patients with:
• Active infections (fungal, bacterial, or viral).
• Uncontrolled diabetes (can cause hyperglycemia).
• Osteoporosis (risk of bone thinning).
• Glaucoma (may increase intraocular pressure).
Potential Side Effects:
• Local skin atrophy or depigmentation (especially in intralesional injections).
• Systemic steroid effects (weight gain, Cushingoid appearance, mood changes).
• Joint damage if overused in intra-articular injections.
3. How Often Can Kenalog Be Given?
• IM Injections: Every 4-6 weeks, if necessary.
• Joint Injections: No more than 3-4 times per year in the same joint.
• Intralesional: Every 4-6 weeks until improvement is seen.
Dilution of Kenalog (Triamcinolone Acetonide) Before Injection
Whether you dilute Kenalog with saline or inject it straight from the vial depends on the indication and route of administration.
1. Intramuscular (IM) Injection
Use undiluted (straight from the vial)
• Kenalog 40 mg/mL or 80 mg/mL is typically used without dilution for systemic anti-inflammatory effects.
• Use a 21-23 gauge needle and inject deep IM into the gluteal muscle.
• Do not inject subcutaneously due to the risk of tissue atrophy.
2. Intra-Articular (Joint) Injection
Can be diluted with saline or lidocaine
• Small joints (fingers, toes, wrists): Dilution may be needed (e.g., 1:1 with 1% lidocaine for comfort).
• Large joints (knees, shoulders, hips): Usually undiluted, but some providers mix with saline or lidocaine (1-2 mL of 1% lidocaine without epinephrine) for pain relief.
• Always aspirate before injection to avoid injecting into a blood vessel.

B. Intra-Articular (Joint) Injection
Indications: Osteoarthritis, rheumatoid arthritis, bursitis, tendinitis.
Common Dose:
• Small joints (fingers, toes, wrists): 5-10 mg
• Medium joints (elbows, knees, shoulders): 20-40 mg
• Large joints (hip, spine): 40-80 mg
Technique:
• Use a 25-27 gauge needle for small joints; 22-25 gauge needle for large joints.
• Inject directly into the joint space under sterile conditions.
• Limit to no more than 3-4 times per year to avoid joint damage.
C. Intralesional (Direct into Skin Lesions)
Indications: Keloids, hypertrophic scars, alopecia areata, cystic acne.
Common Dose:
• Keloids/scars: 10-40 mg/mL
• Alopecia areata: 2.5-10 mg/mL
• Cystic acne: 2.5-5 mg/mL
Technique:
• Use a 30-gauge needle.
• Inject superficially into the lesion, avoiding excess volume to prevent skin atrophy.
• Repeat every 4-6 weeks as needed.
D. Epidural (Off-Label)
Indication: Severe spinal inflammation, herniated discs.
Common Dose: 40-80 mg per injection
Technique: Performed by a trained specialist under fluoroscopy guidance to ensure accuracy and avoid complications.

3. Intralesional (Keloids, Scars, Alopecia Areata, Cystic Acne)
Dilution is recommended
• Kenalog 10-40 mg/mL is often too strong for skin injections, so it should be diluted with saline or lidocaine to prevent tissue atrophy.
• Typical dilution ratio for keloids/scars:
• Kenalog 10 mg/mL → No dilution needed.
• Kenalog 40 mg/mL → Dilute 1:4 with saline or lidocaine (e.g., 0.1 mL Kenalog + 0.4 mL diluent).
• For cystic acne: Use 2.5-5 mg/mL (heavily diluted).
4. Epidural Injection (Specialist-Only Use)
Dilution Required
• Usually mixed with saline or local anesthetic under sterile conditions.
• Only administered by pain specialists under fluoroscopic guidance.
General Dilution Guidelines:
Key Points to Remember:
• For IM and joint injections → undiluted or minimal dilution.
• For skin/scalp injections → always dilute to prevent atrophy.
• Do NOT use lidocaine with epinephrine in joint or scalp injections.
